All youth must be registered with Child & Youth Services (CYS). Registration is simple and free for eligible families - just visit Parent Central Services or sign up on site at the Youth Center.
Required Documents
- Military ID
- Proof of Eligibility- DEERS enrollment, birth certificate, court documents
- Health Assessment and Sports Physical (due within 30 days of registration)
- Immunization Records (required for children under 5 years who have not started school and all homeschooled children)
- Family Care Plan 5305 (required for single/dual military, due within 30 days of signing into your unit)
